In seattle, if you are in downtown free ride zone, you can basically get on board the bus on any doors...but after 7, you can only use the front door. Also, I think alot of US bus drivers dun wanna go through the trouble of asking those fare dodges to pay for the bus fare...(remember Mark Mclaughlin incident 10 years ago, where the King County Metro bus driver was attacked and flew off SR99 Viaduct). Those duty are left to the Transit Police to enforce.
I think the fare enforcement really depends on the city. I live in Portland, OR. The rear door is relatively narrow so you hardly can get off the bus from there either. And the design of the rear door is that you have to push from the inside. So, you really cannot dodge the fare. Also, the fare inspector will do random check on buses or MAX to see if passengers have tickets or not outside Fareless Square. I once was on the bus, and everyone got a fare inspection on Hawthorne Bridge, which is the first stop outside Fareless Square, leaving downtown, in Portland. And guess what, if you do not have a ticket, you will get a hefty fine. As far as I remember, fare inspector rarely shows up in San Francisco.
Yup, I sensed that when I was in PDX too, there are fare inspecter on board of Max and Buses...but in Seattle, that's rarly happen. even on the LINK, i saw it once and 1/2 of the passenger got ticketed because they seems to have no clue on how to pay for the ride. Also, i've been using ORCA card since June, and I found that 1/2 the time, the driver don't even turn on the reader, and just let me go....

Some cities do have that, in Seattle, it's operate by the Bus operator, also, after 7pm the rear door will not be in use for safety reasons and the Downtown free zone will turn to normal fare zone.
